WAC 110-301-0106(10)(a)(b) During the onsite monitor visit this licensor asked for the lead teacher L.M.'s staff file and the site cordinator, M.D., stated they do not have a file onsite yet for him. This licensor observed in the electronic merit system that L.M does not show a current food handlers card. WAC 110-301-0106(8) During the onsite monitor visit this licensor observed that L.M. did not have a Bloodborne Pathogen card in staff file or in the electronic merit system before working with children. WAC 110-301-0106(9) During the onsite monitor visit this licensor observed that L.M. did not have CPR or First Aid card in staff file or in the electronic merit system before being counted in staff to child ratio.
